Accreditation Canada's Case Management Standards help organizations assess the quality of case or care management services provided by a team or organization. Case management is here defined as the process of providing case management services and coordinating case management activities, rather than focusing on case managers specifically.
The standards are based upon five key elements1 of service excellence: leadership, people, process, information, and performance.
These standards contain the following sections:
- Investing in case management services
- Engaging prepared and proactive staff
- Providing safe and appropriate services
- Maintaining accessible and efficient information systems
- Monitoring quality and achieving positive outcomes
