Qmentum uses a variety of methods to collect data and measure organizational performance. Some aspects of care and service are assessed through the questionnaires on standards, while others are assessed using performance measures such as indicators or instruments.
Indicators
Indicators specify the data to be collected related to various aspects of patient/client care and services. Currently, there are indicators for Patient Safety and Hospice Palliative Care.
Patient Safety Indicators
Hospice Palliative Care Indicators
- Access: Clients with access 24/7
- Continuity of care: Use of a standardized pain assessment tool and collaborative care planning
- Pain and symptom burden: Use of the Edmonton Symptom Assessment Scale (ESAS) on admission; average pain burden of clients for whom the ESAS was completed
- Client and family goals: Percentage of client charts documenting client and family goals
Instruments
Instruments are questionnaires used to collect information on critical aspects of health services. There are three Qmentum instruments:
- Governance Functioning Tool - completed by board members
- Patient Safety Culture Tool - completed by staff members
- Worklife Pulse Tool - completed by staff members
